Become a member of our practice and a flat monthly fee will cover ALL primary care visits and services. You'll have direct access to your physician, easier scheduling and longer appointments.


We can see anyone with Medicare or PPO insurance, but we will not be billing insurance for our primary care services.

How Are We Different?

We're a Direct Primary Care (DPC) practice. DPC is a unique approach to healthcare and patients love it. So, how is it different?

Fewer patients

Did you know that most Primary Care Providers have between 2,000-3,500 patients? We limit our practice to 400 or less. That means that your doctor spends more time with you when you need her. Plus, when she is not actively seeing a patient, she is able to help coordinate and support your care.

No insurance

We do not bill any insurance plans so you won’t have a middle-man in your primary care. Decisions about your healthcare will be made by you and your doctor without the impersonal management that comes from third-party payers. Instead, patients pay a monthly membership fee which can save money and covers excellent treatment. Give it a try, and you’ll quickly see for yourself the value of this approach!

Meet the Doctor


Molly Koehler, D.O.

Board Certified Family Physician

I knew I wanted to be a doctor when I was nine years old and our family friend's daughter was diagnosed with leukemia at the age of three. I wanted to be able to care for and connect with people and families when they needed it most. Primary Care gives me that opportunity - to build lasting relationships with my patients and become a trusted health advisor for their family.


I'm eager to bring Direct Primary Care (DPC) to Marin, a model that revitalizes the patient-doctor relationship, provides better access to care, more time with your doctor, and personalized care you trust.


To do this right, time is crucial. In my previous job, with over 2000 patients, I was seeing 17 or more visits a day, often only getting fifteen minutes with each patient, when they needed thirty or sixty minutes. Rushed appointments don't feel good for the patient or for the physician. Daily, my patients would tell me how much they appreciated prompt responses to medical questions and access to urgent visits when they were sick, but even squeezing patients in over lunch and working frantically after hours, it became impossible for me to provide that level of care for that many people.


Additionally, having to write notes and document countless requirements for insurance companies was dominating my day, distracting me from what mattered the most: the person in front of me and their health. In DPC, the patient becomes the center of care again; I can refocus my efforts, documentation, and research on only what helps their care.


Why is it so hard to find a general doctor in Marin? Why is your doctor spending so much time looking at their computer instead of at you during your visit? Why is burnout so high, and only increasing in primary care? The evolution of insurance-driven, fee-for-service care has brought us to an unsustainable state for patients and their providers. DPC is a refreshingly simple, transparent alternative that offers many solutions to these challenges. With DPC, time together is no longer a thing of the past.


As a Marin native, I was happy to return to the area with my family in 2017. This has been my favorite community to work in - with some of the most lovely patients I've had the privilege of caring for and some of my favorite doctors to collaborate with. DPC will also allow me more time with my family, my two growing boys, and my aging parents, some things that have suffered in my recent years of practice. I look forward to being an example of work-life balance and healthy habits for my patients.
